Monthly youth newsletters are filled with activity details and sent monthly.
Please watch for activity sign up deadlines! This helps us plan accordingly for food, transportation, etc. If there is a conflict, just call Jim.

| FORMS |
The churchs goal is to keep your youth safe at all times. To help us to do this, we need two forms for each youth for each trip. Having this information and permission from you will assist us greatly.
Click on the button to download the form to your computer, fill in; then print, fax or mail to Jim at FPCF.

Activity forms will need to be filled out for each off-site or overnight church activity (one form per youth). This form gives us your permission for your youth to participate in an activity, provides a place for any changes to the Medical Release form, and reviews the church rules for you and your youth. Please turn this form in to Jim Pope before each activity. |

A Medical Release form will be valid and on file from September 1, 2007 - August 31, 2008. Please fill this out and turn it in to Jim Pope ASAP. Your youths form will be taken on any off-site activities. |
